Dog Adoption Form

Thank you for choosing to rescue your next family member! The following information is required so that we can assist you in the selection of your new pet. Filling out this form and having a Much Love volunteer come to your home for a homecheck helps us find the animal most compatible with your lifestyle and pair you with a forever friend. After submitting your application, if you do not hear from a Much Love volunteer within 24 hours, please email us at info@muchlove.org.

In order to be considered as an adopter you must:

• Be 25 years of age or older
• If renting, have the knowledge and consent of your landlord
• Be able and willing to spend the time and money necessary to provide training, medical treatment and proper care of the pet.
• Have identification with your current address

If your application and homecheck are approved, you will be able to adopt your new furry friend! We have a contract for you to sign and there is a $350 adoption fee for dogs and $400 for puppies.

All Much Love animals are spayed or neutered, vaccinated, and microchipped before they are adopted.
